Checklist for the Doctor of Physical Therapy Program

All checked items listed below are required for a complete application.  If items are missing, your application may not be processed.

• All prerequisite courses completed by the end of spring semester of the year you wish to enter the program, with a minimum overall grade point average (GPA) of 3.0 and a science/math GPA of 3.0.  Click here for prerequisites.
• All prerequisite courses completed with a grade of “C” or better.
• Complete at least 50 hours (volunteer or paid) observation experience under the general supervision of a licensed physical therapist (see Documentation of Experience below).
• Complete the on-line Texas Common Application.
• Submit the $45.00 application fee: may be paid by credit card (MasterCard or Visa), check, or money order (payable to UTHSCSA). IMPORTANT: Include your application ID on your check or money order. Applications will not be processed without the non-refundable application fee. Fee waiver requests are not accepted.

Submit the following materials and documents to the Registrar at the address above, before the application deadline:

• Official transcripts from every college or university you have attended – If official transcripts are sent to you, you may send them to the Registrar if the seal on the envelope is not broken.
• Two (2) reference forms – Click here to download or print reference forms.
• Personal Statement – A one-page, typed, single-spaced statement that addresses your goal of becoming a physical therapist
• Personal Resume – Including work experience, honors and awards, extracurricular organizations and activities, and community service
• Documentation of Experience – Document at least 50 hours of volunteer or paid observation experience.  Click here to download or print the form.
• Graduate Record Examination scores (GRE) – GRE scores are not currently used in making program acceptance decisions but must be submitted as part of your file. School Code: 6908.
• Request of State Required Prerequisites (if required) – Necessary if you already have a bachelor’s degree but are lacking any the UT Health Science Center Core Curriculum requirements.  If you waive any core curriculum requirements, you will also waive the Bachelor of Science in Health Care Sciences (BSHCS) that is awarded after one year in the Physical Therapy program.  Click here to download or print the waiver form. • Request for Waiver of Program Prerequisites (if required) – Necessary if you wish to request substitution of a program prerequisite course a waiver of program prerequisites.  Include a copy of the course number and title, description, college/university where it was taken, and state which prerequisite that it will substitute for.  Click here to download or print a waiver form.
• TOEFL score (Test of English as a Foreign Language) – Required only of international applicants from countries where English is not the native language; minimum score of 68 (Internet), 220 (computer), or 560 (paper) is required.
• International Transcript Evaluation (international students, only) – Transcripts of coursework taken at colleges/universities outside the United States must be submitted to the Registrar, along with a course-by-course evaluation by an approved credentialing organization.  For more information, click here.
